In Ontario, Advance Care Planning allows individuals to make decisions about the care they would want in the future if they become unable to speak for themselves. This process involves considering personal values, wishes, and rights in healthcare, discussing them with trusted people, and designating a Substitute Decision Maker (SDM) – someone who can speak and make decisions on their behalf if necessary

Jane Meadus is a staff lawyer and the Institutional Advocate at ACE. She represents clients in long-term care homes, hospitals, psychiatric facilities and care homes (retirement homes) with respect to related legal issues.  Jane practiced in the private bar until joining the Advocacy Centre for the Elderly in 1995.    Jane is the author of Chapter 9: “Elder Law: Unlawful Detention of Older Adults in Various Facilities” in the Canadian Anthology on Mental Health and the Law. Jane is a Distinguished Fellow of the Canadian Centre for Elder Law Studies.
